More likely a prototype though, the model number is 6XX-XXXX. From the name, dot cross hatch card, I think this is a stand alone device used to generate test patterns for a monitor. The card has Apple Computer on it, copyright 1986 and what appears to be a 15 pin D socket. My best guess is this was made by Apple for testing the Apple IIgs RGB monitors.
